Students Applying through the SUNY Application

The SUNY supplemental financial form can be filled out on the SUNY Application itself.

Students Applying through the Common Application

Students who have submitted a Common Application can submit their EOP supplemental financial form on the SUNY website.

A signed photocopy of parent’s 1040 income tax form and W2 forms, or if the source of income is from an agency such as Social Security or Social Services, a letter from the agency stating the total amount received in the year prior to admission.

All applicants must submit a Free Application for Federal Student Aid (FAFSA).

All transfer applicants must:

• Submit transcripts from all previously attended colleges.
• Submit verification of their EOP/HEOP/SEEK status at the colleges.
• Attend an EOP information session at Purchase College.
